Physical and Occupational Therapy are related services that are available to students with Individualized Education Programs (IEP) or Service Agreements who need physical therapy and/or occupational therapy to benefit from education.
Services available through the PT/OT Program:
Evaluation of students using appropriate tests, skilled professional observation, and supplementary information from agencies and records to determine the need for
school-based physical therapy/occupational therapy services
Parent and teacher training and support
Individual therapy in schools by licensed therapists
Consultation with school personnel, parents, and physicians about disabilities, development, adaptations and assistive devices
Assistance with the development of IEP or Section 504 Plans, goals and objectives
Completion of ACCESS (medical assistance) forms
To refer a student for a Physical Therapy and/or Occupational Therapy evaluation:
Complete a Physical and Occupational Therapy Referral Form
Please make sure that the "permission to evaluate" section of the referral is signed by the parent and includes the child's physician information
If the parent has submitted a physician's prescription, enclose it with the referral

Please note that if the outcome of a Physical/Occupational Therapy evaluation is a recommendation for services, a physician's prescription must be obtained before services can begin.
